Healthcare Management Administrators (HMA) Selects Lyn Health to Provide Enhanced Care Support for Polychronic Members

New collaborative model will provide HMA members with multiple chronic conditions access to comprehensive and holistic care management and clinical advocacy services.

BELLEVUE – December 15, 2021 – Healthcare Management Administrators (HMA), a pioneer in self-funded health benefit programs and one of the nation’s largest third-party administrators, announced today a new partnership with Lyn Health to make available Lyn’s industry-leading clinical care model for polychronic members.

Lyn Health, founded in 2021, provides human-centric healthcare created for people with multiple chronic conditions, delivering clinical, advocacy and care coordination services via a single point of contact, 24/7. Leveraging virtual capabilities and in-person resources, Lyn improves quality of care and lowers cost for the polychronic population.
